REQUIREMENTS FOR PRINTING FORMS

Forms must meet the following standards as set forth in Rule 84(b)of the Maine Rules of Procedure:

1. Forms with multiple pages must be double-sided.
2. Forms must be printed on 20 lbs. acid-free natural-white 25% cotton fiber paper or better.
3. Forms must be printed with 600 dot per inch black laser printer or better.

NOTES:
1.  Natural-white is not the same thing as bright white.
2.  You will be charged a fee for the forms when filed with the court.
      Forms 1 to 4 pages: $1.00 each; Forms more than 4 pages: $2.00 each

IF THE ABOVE REQUIREMENTS ARE NOT FOLLOWED, YOUR FORMS WILL BE RETURNED TO YOU FOR CORRECTION.
